DSL - INT SYNC Template

This DCL - INT SYNC Template (available in the Attachments below) should be used if the customer reports intermittent connection and is supported by disconnects in Radius.

If the USER-REQUEST in RADIUS outnumber the NAS-REQUESTS, it is best to have a modem swap be completed before escalation to the carrier to help ensure the CPE Modem is not the cause of the issue.

-How often does the connection drop:
-Remove the router:
-Phone cable 5ft or less:
-Tried other phone cable:
-Tried another phone jack:
-Remove all filters or splitters before modem:
-All phones filtered (Regular DSL Only):
-Unplug all devices except modem (Regular DSL Only):
-Any changes (electrical storm, bell work in area, renovations):
-Repeat problem (paste previous ticket #):
